The short answer

A Form 65 Is Your Asbestos Job, Registered With the Regulator

A Form 65 is the official notification a licensed asbestos removalist lodges with SafeWork NSW before carrying out notifiable asbestos work. It tells the regulator who is doing the work, where, when, and under which licence — so the job is on record and accountable before anyone touches a sheet of asbestos.

It isn’t red tape for its own sake. The notification is one of the things that separates a compliant removal from a backyard job — and it’s part of the paper trail that protects you if a council, insurer or future buyer ever asks how the asbestos was handled.

When Is a Form 65 Required?

As a general rule, licensed asbestos removal work is notifiable — meaning it needs a Form 65 lodged before it starts. For Class B (bonded, non-friable) removal, SafeWork NSW requires a minimum of five calendar days’ notice. That timeframe is the single biggest reason we plan your start date early: the notice period runs before the crew can begin, so the sooner it’s lodged, the sooner your job can go ahead.

Whether a specific job is notifiable depends on the material and the work involved. When you book with HazWreck, we assess that as part of the quote and tell you plainly whether your job needs a Form 65 — no guesswork. A whole structure almost always clears the threshold: a fibro garage is well over 10m² of sheeting once you count walls and roof, so asbestos garage and shed removal is notifiable work as a matter of course.

Who Lodges It — You or the Removalist?

The licensed removalist lodges it. Not the homeowner, not the builder. It’s our job, our licence, and our name on the notification. When your removal requires a Form 65, HazWreck prepares and submits it to SafeWork NSW on your behalf — once your deposit is received — so you never have to navigate a government portal or work out what to write.

What a Form 65 Actually Contains

  • The licensed removalist’s details and licence number (ours: SafeWork NSW Class B AD214755)
  • The site address and a description of the asbestos work being done
  • The planned start and finish dates
  • The type of asbestos and how it will be removed and disposed of

Put simply: it’s the job, on the record, before it begins. Combined with the disposal dockets you get on completion, it’s the documented proof that your asbestos was removed lawfully.

How much notice does a Form 65 need?
